This form is used to communicate with City personnel regarding questions, concerns or complaints.

Please note, for a complaint regarding the police department,
a separate form must be completed and submitted to the Zion Police Department.

Police Department Citizen Complaints         

If you witness an officer acting outside of our mission or values, please complete a Citizen’s Complaint Form. After notarizing the form, submit it to the Zion Police Department. A notary public is available on-site Mon-Fri 9-5.
